If you need medication refills, please call your pharmacy first. Due to the spreading threat of COVID -19, respectfully asks that all patients self-screen before arriving at the clinic. If any of the below conditions are true, please contact our office before your appointment to discuss whether or not you should be rescheduled. If you currently have a cough, congestion and/or fever. If you have traveled recently to countries of high risk – China, Italy, Iran, South Korea or Japan. If you have been in close contact with an individual known to have COVID-19. In addition, family members are unable to accompany the patient inside the office unless absolutely necessary. We are also asking that if you do not feel your appointment is urgent at this time (i.e., for a routine annual exam) and you are not currently experiencing any cardiac symptoms, to consider voluntarily rescheduling your appointment. We apologize for any inconvenience this may cause. If you need to schedule an appointment or have cardiac concerns, you may call the office at 541-930-7222 between the hours of 8:30 am-5:00 pm to speak with one of our schedulers or to leave a message for a clinical team member to call you. There are physicians on call after hours for emergencies. Thank you for your patience and understanding.
